At Beaumaris Medical Centre, the doctors and our team of nurses take a team approach to managing chronic disease. Our practice nurses are experienced in all aspects of chronic disease management and will assist your doctor in creating a management plan that is tailored to your specific needs. As a patient you will have onsite access to a range of allied health professionals that can assist in your care and work collaboratively to improve your health. If you have a chronic disease, you may be eligible for participation in the enhanced primary care (EPC) scheme and receive funding towards five allied health sessions per calendar year. We have an ankle brachial index machine to assess your risk of peripheral vascular disease, and our onsite pathology provider can assist with blood tests and ECGs where necessary.
