Our team of nurses and the doctors practising at our clinic are committed to assisting our elderly population to stay in their own homes where this is desired and safe. They perform aged care health assessments each week, where our highly trained practice nurses provide home visits to assess your health, medications, concerns and the safety features of your home. You then see your doctor in the clinic. This collaborative approach allows them to assist you in utilising community services and safety features within your own home to make it safer for you and allow you to enjoy your independence for as long as possible.
Some of the doctors provide medical care to nursing home patients in our local area, allowing patients to continue being seen by their familiar GP who knows the patient and their medical history.
